Care Success Specialist Support Coordination

Level 3 Specialist Support Coordination provides higher-level support for individuals with complex needs, including but not limited to mental health conditions, cognitive impairments, or physical disabilities. It’s designed to help participants manage multiple service providers and overcome significant barriers to accessing the support they need. 

Level 3 is for participants with complex needs who require more intensive support, including crisis planning, managing multiple providers, and navigating challenging situations. Level 2, on the other hand, focuses on helping participants connect to services and manage their NDIS plan more generally. 

Crisis planning helps us work with you to create strategies for handling unexpected situations, so you feel prepared if something goes wrong. We develop a contingency plan that outlines steps to take in challenging times, ensuring you have a roadmap to follow. 

Yes! Level 3 Specialist Support Coordination is covered by your Capacity Building budget, but it is important that Specialist Support Coordination is clearly stated in your plan for it to be funded. If it's not listed, we can assist in requesting a Change of Circumstances to ensure it's included if you require this level of support.
