Level 2 Support Coordination is designed to provide a general level of Support Coordination for participants who may need some help in engaging with services & supports, or even just working out what means what in the NDIS world! This level of support coordinates the delivery of services from various systems or sectors, such as health, social services, education or justice, to ensure all necessary supports are in place.
Level 3 Specialist Support Coordination offers a higher level of intensive Support Coordination for participants with very complex needs, requiring significant assistance to navigate multiple systems and overcome challenging barriers. It focuses on highly individualised plans and accessing supports not typically available through existing service systems as well as closely working along side the NDIS and other services to monitor and implement strategies.
If you're not sure what is funded in your NDIS plan, feel free to drop us a line for a confidential discussion on how we can assist you further.
There are heaps benefits of working with an independent support coordinator! With them, you can expect smaller caseloads, increased flexibility, and a more personalised experience. This is because independent coordinators provide a customised service that caters to your specific needs. They manage their own caseloads and offer tailored support that differs from larger organisations.
Moreover, they have an in-depth understanding of the local support options available and can give you comprehensive information that will help you make informed decisions about your care. They will work hand in hand with you to find services that are suitable for you, enabling you to receive the necessary support to thrive!
Care Success can provide Support Coordination services to individuals who are funded under the National Disability Insurance Scheme in Australia.
There are a few different 'types' of funding - which is where things can get a little confusing! However, rest assured, Care Success can provide services under all funding types which include Agency-Managed, Plan-Managed and Self-Managed budgets.
Care Success is not yet a Registered NDIS Service Provider (we are working on it!) However, Care Success has entered into a sub-contracting collaboration agreement with ARK Support Coordinators, who are a registered NDIS provider. Through this agreement, Care Success is able to provide NDIS supports to Agency-Managed participants.
Yes, Care Success has Individual Professional Indemnity and Liability Insurance.
Yep! if you are Agency-Managed, you can still access NDIS supports through Care Success as they are able to provide Agency-Managed supports under the NDIS Registration of ARK Support Coordinators. NDIS Registration ID# 4050107926.
Absolutely! Care Success strongly believes in choice and control. We will work with you to ensure you have the freedom to choose your preferred service providers, giving you the flexibility and control over your own supports. But, we do know that there are many providers out there and so based on many years of local industry knowledge, we have some pretty good provider recommendations also if you're feeling a little overwhelmed by it all.
We understand that everyone's situation is unique. If you have any further questions, need clarification, or want more information about working with Care Success, please feel free to contact us through either call or text on 0488 070 004 or email steph@caresuccess.com.au !
